• What is DIABETES MODY?

    The research about diabetes is important and here are some facts from one of the research institutes in Sweden, the University in Lund. They explain a form of Diabetes called MODY that differ some from both Diabetes type1 and type2. Learn more:

    MODY (Maturity Onset Diabetes in Young) is a collective name for multiple forms of hereditary diabetes. So far, six different MODY types have been discovered and the common cause for them is that the genetic cause of the disease is known.

    All forms of diabetes have hereditary elements, but for example for type 1 and type 2 diabetes, there are several genes in combination with environmental factors behind. At MODY the error is in a single gene.

    MODY is the strongest hereditary diabetic disease, which means that it occurs for several generations in the same family. A child with a parent with MODY has 50 percent risk of suffering from it. Chance determines which children will and who will not inherit the recombination. It is also common that there are other types of diabetes at the same time.

    In general, MODY debuts before the age of 25 and the disease can best be described as a form of type 2 diabetes in young people.

    A common feature of MODY is the lack of insulin that causes blood glucose levels to be too high. However, patients have no real insulin resistance, which is common in type 2 diabetes. MODY, on the other hand, is characterized by high insulin sensitivity, which increases the risk of low blood sugar levels when patients are treated with insulin or pills that enhance insulin production.

    Nor are other typical metabolic disorders, such as obesity, blood fatigue or hypertension common. Complications from the major blood vessels in the form of cardiovascular disease are rare.

    Since most MODY patients are young and normal-weighted, it is not uncommon for them to be diagnosed with type 1 diabetes. With MODY, there are no antibodies to the insulin-producing cells, so it is not, as with type 1 diabetes or LADA, an autoimmune disease.

    Despite the similarities between the different MODY forms, such as insulin deficiency, they also have important features depending on the gene mutation behind. MODY 1 and MODY 3, the most common form in Sweden with almost half of all MODY patients, requires treatment, often with insulin injections. Pretty moderate increases in blood sugar levels lead to an increased risk of diabetic complications in the form of eye and kidney damage.

    MODY 2, on the other hand, is mild and complications are rare. The disease requires no treatment. The genus mutation that causes MODY 2 was discovered 30 years ago. The gene controls the transport of glucose from the blood into the insulin-producing beta cells. This is a signal to the beta cells to release insulin to the blood. At MODY 2, the signal is weakened, which causes too little insulin to be released and blood sugar rises above normal levels. MODY 4, 5 and 6 are unusual. In addition, there are probably still unknown MODY forms, as approximately 20 percent of patients have no known gene mutations.

    The research on MODY has led to important insights of how different genes participate in the regulation of the body’s sugar and energy balance. This knowledge contribute to a better understanding of the mechanisms behind other forms of diabetes.

    Up to five percent of all forms of diabetes belongs to any of the MODY modes.

    Forskningen inom diabetes går framåt och är viktig. Här kommer intressant rapport från Lunds universitet, som förklarar vad diabetesformen MODY är.

    MODY (Maturity Onset Diabetes in Young) är ett samlingsnamn för flera former av ärftlig diabetes. Hittills är sex olika MODY-typer upptäckta och det gemensamma för dem är att den genetiska orsaken till sjukdomen är känd.

    Alla former av diabetes har ärftliga inslag men vid till exempel typ 1 och typ 2 diabetes är det flera gener i kombination med faktorer i miljön som ligger bakom. Vid MODY sitter felet i en enda gen.

    Hälften av barnen insjuknar

    MODY är den starkast ärftliga diabetessjukdomen vilket innebär att den förekommer i flera generationer i samma familj. Ett barn till en förälder med MODY har 50 procents risk att drabbas. Slumpen avgör vilka barn som ska och vilka som inte ska ärva genkombinationen. Det är inte heller ovanligt att det samtidigt finns andra diabetesformer i familjen.

    Liknar typ 2 diabetes

    I regel debuterar MODY före 25-årsålder och sjukdomarna kan bäst beskrivas som en form av typ 2 diabetes hos unga.

    Ett gemensamt drag för MODY-formerna är bristen på insulin som gör att blodsockerhalten blir för hög. Däremot har patienterna inte någon uttalad insulinresistens, vilket är vanligt vid typ 2 diabetes. MODY karaktäriseras tvärtom av hög insulinkänslighet vilket ökar risken för alltför djupa blodsockerfall när patienterna behandlas med insulin eller tabletter som driver på insulinproduktionen.

    Risk för felaktig diagnos

    Inte heller andra typiska störningar i ämnesomsättningen, till exempel övervikt, blodfettsrubbningar eller högt blodtryck, hör till bilden. Komplikationer från de stora blodkärlen i form av hjärt- och kärlsjukdomar är sällsynta

    Eftersom de flesta MODY-patienter är unga och normalviktiga är det inte ovanligt att de får diagnosen typ 1 diabetes. Vid MODY finns det inga antikroppar mot de insulinproducerande cellerna, det är alltså inte, som vid typ 1 diabetes eller LADA, en autoimmun sjukdom.

    Avgörande skillnader

    Trots likheterna mellan de olika MODY-formerna, exempelvis insulinbristen, har de också viktiga särdrag beroende på vilken genmutation som ligger bakom.

    MODY 1 och MODY 3, den i Sverige vanligaste formen med nästan hälften av alla MODY-patienter, kräver behandling, ofta med insulininjektioner. Ganska måttliga förhöjningar av blodsockerhalten leder till en ökad risk för diabeteskomplikationer i form av ögon- och njurskador.

    MODY 2 däremot är lindrig och komplikationer är sällsynta. Sjukdomen kräver ingen behandling.

    Försvagad signal

    Genmutationen som orsakar MODY 2 upptäcktes för 30 år sedan. Genen reglerar transporten av glukos från blodet in i de insulinproducerande betacellerna. Detta är en signal till betacellerna att frisätta insulin ut till blodet. Vid MODY 2 är signalen försvagad vilket gör att för liten mängd insulin frisätts och blodsockret stiger över normala nivåer.

    MODY 4, 5 och 6 är ovanliga. Dessutom finns det förmodligen fortfarande okända MODY-former då ungefär 20 procent av patienterna inte har någon av de kända genmutationerna.

    Nya kunskaper

    Forskningen om MODY har lett till viktiga insikter i hur olika gener deltar i regleringen av kroppens socker- och energibalans. Dessa kunskaper bidrar till en bättre förståelse av mekanismerna bakom också andra diabetesformer.

    Högst fem procent av alla diabetes tillhör någon av de olika MODY-formerna.
